- Go to www.mayoclinichealthsystem.org
- Enter your Mayo Clinic number, activation code, email address, name, and date of birth
- Create security questions, a username, and password
- Click “Submit”
And that’s it! Your account will be ready to use immediately.
Logging In to Your Account
To sign in:
- Go to the Mayo Patient Portal login page
- Enter your username and password
- Click “Sign In”
Resetting Your Mayo Portal Password
If you ever forget your password, simply:
- Go to the Mayo Patient Portal login page
- Click “I need help with logging in”
