Activating your card is easy. Just follow these simple steps:

- Your My Health Pays Reward Card
- Date of Birth
- ZIP Code

- Open your web browser and navigate to the My Health Pays Rewards activation page: www.myhealthpaysrewards.com/activate
- Click on “Activate” below “Click here to activate your card”
- Enter your Member ID number located on the front of your card
- Input your Date of Birth in MM/DD/YYYY format
- Enter your ZIP code
- Create a 4-digit PIN to use with your card
- Complete the activation process by following the remaining prompts
And that’s it! Your My Health Pays Reward Card is now active and ready for use.
Note: This card cannot be used everywhere Visa cards are accepted and has restrictions on certain purchases like alcohol, tobacco, guns, and ammunition.
